Drone warfare is rapidly reshaping modern conflict, with profound consequences for civilian protection, emergency medical response, and health system resilience. This report examines how the widespread use of drones in Ukraine has transformed patterns of injury, disrupted medical evacuation, and undermined access to health care, despite protections under international humanitarian law. Drawing on MedGlobal’s medical missions, clinical training programs, and partnerships with Ukrainian hospitals and universities from 2022 to 2025, the report documents the rise of complex polytrauma injuries, delayed evacuations due to targeted attacks on ambulances and first responders and increasing rates of infection and psychological distress among civilians. The analysis highlights how Ukrainian health providers have been forced to adapt through decentralized models of care, mobile clinics, underground stabilization points, and expanded trauma, surgical, and emergency preparedness training. Ukraine’s experience offers critical lessons for health systems worldwide as drone warfare becomes increasingly common, underscoring the urgent need for strengthened frontline medical capacity, protection of health services, and preparedness for the evolving realities of contemporary warfare.
