Important points: bloody diarrhoea and URTI in Dili, acute jaundice syndrome in Batugade and World AIDS day.
Formal report:
During week 47 of 1999, reports concerning 6,632 patients seen in 25 health facilities (including mobile clinics) were received at the WHO Office in Dili. Just 5 districts were represented in the data reported by 8 agencies among 15 providing health care services. The districts reporting were Dili, Ambeno, Liquica, Bobonaro and Suai. The ICRC Hospital has started reporting.
72% of the consultations reported concerned communicable diseases. The commonest were upper respiratory tract infection/TB and suspected malaria/fever (each approximately 21% of the total cases of communicable diseases reported). High numbers of URTI/TB cases were reported in Dili, corresponding to 78% of all cases reported this week. This does not relate to the frequency distribution of consultations (Dili accounts for only 60% of consultations) and might also reflect patients searching for TB treatment in the capital, since some health agencies have begun to offer it.
Measles continues to be reported B 26 cases in Dili. No reports were received from Ermera district, but it is likely to have cases registered. The frequency distribution of bloody diarrhoea cases reported continues to be disproportionately higher in Dili this week. The majority of cases were reported from hospital facilities (ICRC, Hospital Português and Bairro Pite Clinic).
Informal reports:
The French hospital laboratory has isolated Salmonella paratyphi A this week from a child with diarrhoea. No other cases related to the patient (in place and time) were reported. Twelve cases of acute jaundice syndrome were informally reported coming from a small village near Batugade. Investigation will take place via TimorAid; material and instructions for specimen collection have been provided and a laboratory in Darwin is ready to receive the samples.
In East Timor between 2,500 and 3,000 blood donation samples were screened for HIV in 94 and 95 and only one HIV positive was found. A Timorese doctor has reported informally that a similar survey was done in 97 and one other HIV positive sample was identified.
