EXECUTIVE SUMMARY

In just two years, the conflict between the Sudanese Armed Forces (SAF) and Rapid Support Forces (RSF) has created the world’s largest humanitarian crisis. Over 30 million people require humanitarian aid, and more than 14 million have been displaced.

El Fasher – the capital of North Darfur and the last major city in Darfur outside of RSF control – is a key flash point in the conflict.

For 500 days, civilians there have been exposed to repeated and severe breaches of international humanitarian law (IHL), including siege warfare and indiscriminate attacks as SAF and RSF vie for control. RSF controls the periphery of El Fasher and has cut supply routes, leading to severe shortages of food, water and medicine in the city. Around 600,000 people have fled from El Fasher and nearby camps since April 2023.

Some of those newly displaced fled to Ad Dabbah, Northern State, where MedGlobal conducted a Rapid Multi-Sector Needs Survey, malnutrition screening, and interviews between September 15–20, 2025. The survey took place in the neighborhoods of Al-Afadh and Housh Maliit. Multiple layers of data validation and quality assurance were applied to ensure reliability of the analysis. The findings offer an alarming glimpse into life under siege for those still trapped in the city, and a stark account of the dangerous and desperate journeys people have undertaken to escape.
