UNDRO SITUATION REPORT NO. 1
02 APRIL 1985

SITUATION

1. UNDRO/UNDP RESIDENT REPRESENTATIVE REPORTS SEVERE OUTBREAK OF CHOLERA EPIDEMIC AMONG REFUGEES IN NORTHWEST SOMALIA. TOTAL NUMBER DEAD REPORTED SO FAR CLOSE TO 400 MOSTLY CHILDREN. MAJORITY OF CASUALTIES IN GANNET CAMP SITUATED CLOSE TO HARGEISA TOWN WITH POPULATION OF 46,000 (OUT OF 481 CASES 375 DIED). CASES ALSO REPORTED IN OTHER NEARBY REFUGEE CAMP AND AMONG LOCAL POPULATION IN TOWN HARGEISA. TOTAL POPULATION THIS AREA 300,000.

ACTION

2. GOVT HAS DECLARED EMERGENCY AND HAS REQUESTED UNDRO TO LAUNCH AN APPEAL FOR ASSISTANCE TO INTERNATIONAL COMMUNITY. UNDRO DELEGATE TO ASSIST IN ASSESSMENT OF NEEDS AND LOCAL COORDINATION RELIEF, EN ROUTE.

3. PRINCIPAL IMMEDIATE NEED IS FOR 20-50,000 LITRES OF INTRAVENEOUS FLUID (EITHER RINGER LACTATE OR NORMAL SALINE CONC. 9/1000 PLUS 10CC SODIUM BICARBONATE: VALUE APPROX. 2 DOLLARS PER LITRE) TO BE FLOWN TO BERBERA, SOMALIA FOR DELIVERY HARGESIA. USAGE RATE OF INTRAVENEOUS FLUID EXTREMELY HIGH: 75 PERCENT ALL CASES REQUIRE INTRAVENEOUS FLUIDS AT RATE OF 20 LITRES PER DAY.

4. UNICEF, UNHCR, ICRC, LRCS AND BILATERALS ALREADY EXPEDITING MEDICAL SUPPLIES INCLUDING INTRAVENEOUS FLUID.

SITUATION

1. IN VIEW TRANSPORTATION DIFFICULTIES DUE LONG DISTANCE MOGADISCIO/HARGEISA, DONORS ARE REQUESTED TO AIRLIFT ALL SUPPLIES TO BERBERA WHERE AIRPORT HAS CAPACITY TO RECEIVE LARGE SIZE CARGO AIRCRAFT. BERBERA 200 KM FROM AFFECTED AREA WITH GOOD AND ACCESSIBLE ROADS. HARGEISA AIRPORT REPORTED CLOSED.

INTERNATIONAL RESPONSE

2. WHO HAS FIELDED A TEAM OF ONE EPIDEMIOLOGIST, ONE MICROBIOLOGIST, NURSES AND SANITARIANS ON SITE FOR EMERGENCY RELIEF AND CLOSE MONITORING SITUATION.

3. 20,000 LITRES OF INTRAVENEOUS FLUID AND OTHER MEDICAL EQUIPMENT CONTRIBUTED BY UNICEF AND TRANSPORTED BY USA PLANE TO ARRIVE BERBERA MORNING 4 APRIL 1985.

4. MSF BELGIUM AND NETHERLANDS LAUNCHING APPEAL FOR A THREE MONTH RELIEF PROGRAMME VALUE US DLRS 375,000. A BELGIAN MILITARY AIRCRAFT SCHEDULED LEAVE ON SATURDAY CARRYING MSF MEDICAL TEAM OF 7 PEOPLE, 2 CARS, PLUS 7,000 LITRES OF IV-FLUID AND FURTHER MEDICAL SUPPLIES. AN ADDITIONAL MSF MEDICAL TEAM ON STANDBY.

5. LRCS INFORMS THAT A JOINT GERMAN (FED.REP.)/NORWEGIAN TEAM (1PHYSICIAN, 4 NURSES) WILL REACH HARGEISA ON MORNING 4 APRIL WITH 12 MT MEDICAL SUPPLIES AND OTHER EQUIPMENT INCLUDING 4,000
LITRES IV-FLUID AND 2 RADIO SETS. FURTHER DELIVERY MEDICAMENTS PROVIDED BY LRCS (INCLUDING 8,000 LITRES IV-FLUID) EXPECTED TO ARRIVE MOGASDISCIO TONIGHT FROM NAIROBI.

1. UNDRO/UNDP RESIDENT REPRESENTATIVE MOGADISCIO INFORMS HARGEISA SEALED OFF WITH NO MOVEMENT PERSONNEL EXCEPT MEDICAL AND RELIEF STAFF. TOTAL NUMBER OF DEATHS ROSE TO 700. DANGER OF EPIDEMIC SPREADING INTO HARGEISA TOWN AND SURROUNDING AREAS (300,000 TO
400,000 INHABITANTS) PERSISTS. SECOND SMALL OUTBREAK OF DIARRHOEAL DISEASE REPORTED IN KISMAYA, SOUTH SOMALIA. WHO STRONGLY RECOMMENDS ALL PERSONNEL ARRIVING IN MOGADISCIO TO COME WITH ADEQUATE SUPPLIES OF TETRACYCLINE OR CONTRIMOXAZOLE.

SITUATION

1. UNDRO DELEGATE REPORTS A TOTAL OF 3,078 CHOLERA CASES AS AT 8 APRIL WITH 873 DEATHS AND APPROXIMATELY 1,500 PERSONS ISOLATED. ACCORDING TO WHO SITUATION HAS IMPROVED SINCE 6 APRIL WITH LESS NEW CASES PARTICULARLY IN GANNET CAMP, AND FEWER DEATHS. IN LOWER-JUBA, SOUTHERN SOMALIA, 4 CASES OF CHOLERA WERE CONFIRMED AND FURTHER 16 SUSPECTED.

INTERNATIONAL RESPONSE

2. SITUATION IMPROVING LARGELY DUE TO PROMPT ARRIVAL NATIONAL AND INTERNATIONAL MEDICAL AND PERSONNEL ASSISTANCE.

3. UNDRO DELEGATE IS ASSISTING IN ASSESSMENT OF NEEDS AND COORDINATION OF LOCAL RELIEF OPERATIONS. TWO LRCS TEAMS ARRIVED IN AFFECTED AREA AND ALREADY OPERATIONAL. MSF PERSONNEL AND SUPPLIES REACHED HARGEISA. WHO HAS FIELDED EXPERT IN CLINICAL MANAGEMENT OF CHOLERA. EMERGENCY HOSPITAL ESTABLISHED 12 KM OUTSIDE HARGEISA.

4. MINISTRY OF HEALTH ADVISES NO ADDITIONAL MEDICAL TEAMS NEEDED AT PRESENT.

5. LRCS WILL SHORTLY LAUNCH APPEAL FOR APPROXIMATELY SFR 2 MILLION FOR CHOLERA RELIEF AND PREVENTION COVERING 6-MONTH PERIOD. THIS WILL INCLUDE SUPPLEMENTARY FEEDING PROGRAMME, MEDICAL SCREENING OF NEW ARRIVALS, AND ASSISTANCE IN TRANSPORT OF WATER.

SITUATION

1. ON 14 APRIL MINISTRY OF HEALTH REPORTED 105 NEW CASES AND 22 DEATHS OVER TWO DAYS PERIOD IN GANNET, HARGEISA, LAS DHURCH, TOGWAJALE AND SEVERAL OTHER REFUGEE CAMPS. CUMULATIVE NUMBER OF CASES SINCE OUTBREAK NOW 3,341, DEATHS 945 AND 1,785 QUARANTINED PERSONS.

2. COMBINED NATIONAL AND INTERNATIONAL EFFORTS HOWEVER HAVE GRADUALLY BROUGHT ABOUT CONTAINMENT OF EPIDEMIC. AUTHORITIES HAVE ESTABLISHED NEW CAMP GANNET B, 3 KM FROM OLD SITE AND MOVED ALREADY 10,000 REFUGEES. MEDICAL ASSISTANCE IN GANNET B PROVIDED BY INTERNATIONAL TEAMS.

3. DAILY REFUGEE INFLUX INTO NORTHWEST FROM CHOLERA SUSPECTED AREAS ESTIMATED AT 200 TO 300. GOVT NOW BUILDING HOLDING POINTS ALONG BORDER FOR MEDICAL CHECKS ON NEW ARRIVALS. LRCS INTENDING TO PROVIDE FOOD, MEDICAL AND PERSONNEL ASSISTANCE AS PART OF ITS SIX-MONTH PLANNED CHOLERA PREVENTION PROGRAMME, (UNDRO SITREP NO.4, PARA 5).

INTERNATIONAL RESPONSE

4. AT REGULAR UNDP CHAIRED DONOR MEETING ON 14 APRIL (ATTENDED BY ALL UN AGENCIES, THE WORLD BANK, EEC AND REPRESENTATIVES BILATERAL DONORS FROM CHINA, FRANCE, ITALY, US) WHO AND UNDRO SUMMARIZED CHOLERA RELIEF OPERATIONS. SUFFICIENCY MEDICAL SUPPLIES AND PERSONNEL AGAIN CONFIRMED.

FUTURE REQUIREMENTS

5. DURING THIS MEETING NEED FOR MEDICAL PREPAREDNESS EMPHASIZED TO ALLOW GOVERNMENT TO COPE IMMEDIATELY WITH SIMILAR OUTBREAK. REQUIREMENTS INCLUDED ESTABLISHMENT SUFFICIENT STOCKS, APPROX. 3 MONTHS SUPPLY ESSENTIAL MEDICINES, STORAGE FACILITIES, SAFE WATER SUPPLY IN AFFECTED AREAS WHICH IMPERATIVE IN VIEW OF LIKELIHOOD NEW RAINS.

6. MINISTRY OF HEALTH PLANNING TO HOLD CHOLERA MEETING WITH UN AGENCIES, DONORS AND NGOS LATER IN WEEK BEGINNING 15 APRIL. MEETING TO REVIEW OVERALL SITUATION AND DISCUSS DETAILS LIST OF MEDICAL REQUIREMENTS FOR CHOLERA PREPAREDNESS AND PREVENTION PHASE. UNDRO WILL COMMUNICATE LIST AND APPEAL FOR ASSISTANCE.

1. UNDRO/UNDP RESIDENT CO-ORDINATOR REPORTS FOR PERIOD 15 TO 22 APRIL 1985 INCLUSIVE 355 NEW CASES AND 49 DEATHS. CUMULATIVE NUM- NUMBER OF CASES SINCE OUTBREAK 3,794, WITH 1,015 DEATHS.

2. SINCE DEPARTURE UNDRO DELEGATE 16 APRIL SITUATION FURTHER STABILIZED. EVACUATION REFUGEE POPULATION OF GANNET A IN HARGEISA TO NEW CAMP GANNET B ALMOST COMPLETED. NATIONAL AND INTERNATIONAL MEDICAL TEAMS CONTINUE TO PROVIDE TREATMENT OF CHOLERA CASES IN GANNET B, TOG WAJALE AND OTHER REFUGEE CAMPS NORTH WESTERN REGION. SUPPLEMENTARY FEEDING AND WATER SANITATION PROGRAMMES HAVE STARTED. HEALTH CONTROL CENTRES BEING SET UP IN GANNET B AND IN TOG WAJALE FOR MEDICAL SCREENING OF REFUGEES WHO CONTINUE TO ARRIVE FROM ETHIOPIA AT A RATE OF 200-300 PER DAY. MEASLE IMMUNIZATION CAMPAIGN ALSO CARRIED OUT AMONG NEW ARRIVALS.

3. MEDICAL SUPPLIES ACTUALLY AVAILABLE IN SOMALIA SUFFICIENT FOR CURRENT NEEDS.

4. AS ANNOUNCED UNDRO SITREP NO. 6 PARA. 6, DONORS MEETING HELD ON 24 APRIL AND DISCUSSED MEDICAL REQUIREMENTS FOR CHOLERA PREPAREDNESS AND PREVENTION PHASE. REQUIREMENTS INDICATED IN FOLLOWING LIST WOULD ALLOW ESTABLISH STOCKS IN MOGADISCIO AND HARGEISA
FOR PREVENTIVE MEASURES AND RAPID INTERVENTION IN CASE OF ANOTHER CHOLERA OUTBREAK. NEEDS HAVE BEEN ESTIMATED ON ASSUMING POSSIBILITY ANOTHER OUTBREAK APPROXIMATELY TWICE THE SCALE OF HARGEISA EPIDEMIC AND FOR A PERIOD OF ONE MONTH.
