1.This template is to be used by any staff assigned by organisations operating in the Tigray Humanitarian Response to transfer accountability data from their organisation`s database or records to the AAP collective platform by the last day of each month.
2.Data from any organisation`s database or record must be anonymised first for the purpose of sharing it for the collective platform so that it cannot be traced to original people or groups providing feedback or complaints. This involves removing actual names and other identifying particulars including address or phone contacts.
3.Essentially organisations should create a duplicate database of its feedback register in the same format as this collective platform`s data points so that each month when they submit they send something showing old and new records whereby old records will indicate latest progress of responding to the feedback.
4.For effective collation of an organisation's feedback data there is need at the organisation's level to update data in the collective template from the original database at least once a week.
