The Afghan Placement and Assistance (APA) Senior Health Case Worker will have a blended role of providing direct service, case coordination, and supervision oversight. As a leadership position on the APA team, this position will assist in file review, grant reporting and monitoring, along with back-up support to the APA Supervisor. The Senior Health Case Worker will have a reduced caseload, with primary responsibility in cases of significant health vulnerabilities or barriers along with cases with attached minors needing case management support for guardianship. This position will be responsible in coordinating initial medical appointments for newly arrived APA clients needing immediate medical assistance by working closely with caseworkers, the Maricopa Health Department, and other providers. The APA Senior Health Caseworker will work closely with the IRC’s Health and Wellness staff to develop new partnerships with medical providers in order to enhance medical access for APA clients.
Specialized Case Management and Coordination:
· Coordinate initial services for assigned APA cases including those with urgent medical needs and attached/ unaccompanied minor cases with guardianship needs.
· Provide specialized case management services to eligible clients, including developing self-sufficiency strategies, monitoring progress, and providing support to clients by addressing housing, health, education, financial assistance, legal issues, and other needs which may emerge.
· Maintain accurate and up-to date case files for all assigned clients.
· Coordinate with the APA team, other IRC programs, and other service providers to meet the needs of clients.
· Coordinate referrals for further specialized services.
Health Care Coordination, and Partnership Development:
· Outreach to medical providers to enhance health care and provision options for APA clients.
· Coordinate with the Health and Wellness team to arrange medical appointments for new arrivals needing immediate medical assistance.
· Work closely with Maricopa Health Department to coordinate medical screenings, vaccinations, and follow-up care for new arrivals.
· Provide specialized assistance and advocacy for clients needing assistance in accessing health care coverage which may not be within Medicaid.
· Ensure appropriate documentation, records, and tracking tools for program case files and reporting. Create tools as necessary for staff and conduct regular case file audits.
· Participate in monitoring and evaluation of programs within the APA Casework program.
· Track spending and assistance for clients, ensure donor deliverables are met, compile data and draft reports as required by donor and agency specifications.
· Provide leadership for Resettlement Caseworkers including program coordination and management, staff supervision, reporting, and grant compliance.
· Assist in on-boarding and training for new hires and interns as needed.
· Monitor ongoing progress of outcomes for clients with caseworkers assigned.
· Conducting performance reviews with Caseworkers on their goals and work progress.
· Provide case coordination and conduct staffing for cases and clients with significant barriers to self-sufficiency.
· Provide back-up support and coverage to APA Supervisor when needed.
· Provide recommendations for program and service improvements.
· Ensure service provision is trauma-informed, equitable, and gender sensitive.
· Follow all IRC policies, procedures and protocols including adherence to the IRC Way and Child Safeguarding.
· Be a leader in APA case management meetings, providing updates and ongoing training on new health measures and protocols.

Position directly supervises: **2-3 staff within the Resettlement Casework Team along with 1-3 interns
Indirect Reporting: APA Program Manager/Deputy Director**
Other Internal and/or external contacts:**
Internal: IRC staff, interns, and volunteers
External: Community partners, religious and faith-based communities, DES, law enforcement, schools, Dignity Refugee Health Partnership, Maricopa Health Department, Arizona State Refugee Health Coordinator, and other medical providers.
· Undergraduate degree in social work or related field or equivalent work experience.
· Two+ years of professional work experience; preferably in a non-profit or social services environment or in role of supervision.
· A minimum of one-year direct casework experience, with some supervisory or staff oversight experience preferred.
· Proficiency in written and spoken English, bilingual ability preferred in Dari and/or Pashto.
· Cross-cultural understanding and strong communication skills, both written and verbal.
· Lived experience as refugee or immigrant. In lieu of lived experience, experience working with refugees and/or immigrants or experience working cross-culturally
· Ability to prepare accurate and timely reports, document client interactions and services and maintain updated and accurate case files.
· Computer proficiency with an emphasis on Microsoft Office applications (Word, Excel, Teams, Outlook). Proven experience in web-based data entry systems, with database experience strongly preferred.
· Highly organized self-starter with proven success prioritizing multiple tasks, making critical decisions, and delivering results in a fast-paced environment with stringent deadlines
· Strong attention to detail with the ability to prioritize and manage time effectively.
· Must have a valid driver’s license, active insurance policy, access to reliable transportation and the ability to travel regularly throughout the service delivery area for home visits, and visits to other agencies or service providers.
· In accordance with Arizona State Law, must possess or be eligible to receive a fingerprint clearance card.
· A combination of standard office environment, remote work, and ‘field’ time within the service delivery area to perform the above outlined responsibilities.
· May require weekend and/or evening work.
