Pacific: Dengue Outbreak - Feb 2025
Disaster description
Between 1 January and 1 December 2025, a total 20,717 of dengue confirmed cases, and 21 deaths have been reported across the Pacific. Pacific Syndromic Surveillance System has reported a total of 39,245 Dengue-like Illness (DLI) cases to date, reaching its highest weekly peak since 2016, with 2,249 cases recorded per week in August. The most affected countries—Fiji, French Polynesia, Samoa, and Tonga—account for 94% of confirmed cases. As of 1 December 2025: continued circulation of dengue remains in American Samoa, Cook Islands, Kiribati, Samoa, and Tuvalu; Cook Islands is experiencing a continued upward trend, with confirmed cases rising 22% in weeks 44–47 compared to the previous four‑week period; Kiribati, Samoa, and Tuvalu are showing a downward trend, with declining case numbers and reduced hospital admissions; French Polynesia declared the end of its two‑year surge on November 2025; The Governments of Fiji and Tonga declared the end of their dengue outbreaks in August 2025. American Samoa
On 8 July 2025, the American Samoa Department of Health declared a public health emergency due to a dengue fever outbreak. As of 10 July 2025, there have been a total of 9 laboratory-confirmed cases of dengue fever reported, of which eight are males and one female. Eight of these individuals required hospitalisation but remain in stable condition. Majority of the cases are occurring in children below 18years of age and three of which are adults. Out of the nine dengue cases reported, seven are imported and two are locally acquired cases with no recent travel history. The initial six cases were serotyped at the Hawaii State Lab, five were Dengue Virus Type 1 (DENV-1) and one was identified as Dengue Virus Type 2 (DENV-2). (Govt. of American Samoa, 15 Jul 2025)

As of 8 August 2025, there have been a total of 34 laboratory-confirmed cases reported of which 28 are males and 6 females. Of the total cases, twenty-six of these cases are travel-related with links to Samoa while eight are locally acquired. The majority of cases are from the 11-16 years age group, followed by 16-20 years and 21-30 years. Clinically 28 patients have required hospital admission at some stage of their illness. All have since been discharged with no dengue haemorrhagic fever or other severe complications reported. Tafuna remains the most affected village, with other areas including Iliili, Leone, Malaimi, Nuli and Laulii. On 8 July 2025, American Samoa declared a public health emergency in response to a dengue outbreak, with 66 lab-confirmed cases reported by 20 August. Cook Islands
The Ministry of Health has officially declared a dengue fever outbreak in Rarotonga on 22 May 2025, following confirmation of two new cases since 19 May. As of 22 May 2025, a total of 7 locally acquired dengue cases have been reported in the past 18 days reaching the outbreak threshold. Since February 2025, a total of eleven isolated cases have been recorded, with Tupapa recording the highest number of dengue cases, five out of eleven cases. Local transmission has been confirmed. The serotypes that are responsible for this current outbreak are Dengue Virus Type 1 (DENV-1) and Dengue Virus Type 2 (DENV-2). (Govt. of Cook Islands, 27 May 2025)

Since February, there have been 50 dengue cases reported. Of these, 44 individuals have fully recovered while 6 cases remain active. Twelve cases required hospitalisation with 10 now discharged. Of the twenty-seven samples with confirmed serotypes, twenty-four were Dengue Virus Type 1 (DENV-1) and three were Dengue Virus Type 2 (DENV-2), indicating co-circulation of DENV-1 and DENV-2. On 22 May 2025, the Cook Islands Ministry of Health declared a dengue outbreak in Rarotonga, with 149 cases reported by 17 September, 130 recovered and 19 still active. Most of the cases belong to the 10-19 age group with no fatalities or severe dengue cases reported. Of 50 serotyped samples, 47 were DENV-1 and 3 were DENV-2, confirming co-circulation. Fiji
As of 03 February 2025, The Ministry of Health and Medical Services has declared a dengue fever outbreak in the Western Division due to a significant rise in cases above expected levels. Since the beginning of the year, 200 cases have been reported, primarily affecting individuals aged 10-29 years. In response, a Dengue Action Plan has been implemented, with ongoing monitoring by divisional heads and senior managers. (Govt. of Fiji, 4 Feb 2025)
As of 23 March 2025, the Ministry of Health and Medical Services reported 5,128 cases of dengue nationally with 2841 cases in the Western division, 1095 cases in the North, 1090 cases in Central and 102 cases in the Eastern division. Additionally, there has been 1,511 admissions nationwide with 4 deaths reported. In addition to the ongoing outbreak that was declared in the Western division on 03 February 2025, the Ministry of Health and Medical Services have since declared a dengue fever outbreak in the Central Division on 11 April 2025 with the most recent increase of cases in March seen to be above the expected levels for the same period last year. The surge in cases have been observed over the last three weeks in the subdivisions within Central while there has also been an increase in admissions of dengue fever cases at the subdivisional hospitals and at the main tertiary hospital. As of 21 May 2025, the Ministry of Health and Medical Services reported 8,708 cases of dengue nationally with 4,357 cases in the Western division, 2092 cases in the Central, 2048 cases in Northern and 211 cases in the Eastern division. Additionally, there has been 1,768 admissions nationwide with more cases expected as health facilities continue to report cases. French Polynesia
In epi week 37, 5 confirmed cases of Dengue Fever were reported (compared to 13, 19, 16 and 18 confirmed cases in epi weeks 35, 34, 33 and 32), with 2 hospitalizations (compared to 0, 0, 0 and 1 hospitalizations in epi weeks 35, 34, 33 and 32) and 0 deaths (compared to 0, 0, 0 and 0 deaths in epi weeks 35, 34, 33 and 32). In EpiWeek 39, 10 new dengue cases (six confirmed and four probable) were reported. Since November 27, 2023, a total of 2,636 cases has been recorded, including 2,358 confirmed and 278 probable cases. No hospitalisations reported in EpiWeek 39. The French Polynesia Health Department announced in its latest surveillance bulletin the end of the dengue epidemic. In EpiWeek 44, 3 new confirmed dengue cases were reported. Guam
The Department of Public Health and Social Services (DPHSS) received one (1) laboratory confirmed Dengue fever report, September 18, 2025. This is the first confirmed dengue case for 2025. Through epidemiologic investigation, DPHSS has determined that this is most likely an imported case. (Govt. of Guam, 22 Sep 2025)
Kiribati
The Ministry of Health confirmed a dengue outbreak on 24 April 2025, after a significant rise in confirmed cases. As of 12 May 2025, the Health Ministry reported a total of 51 dengue confirmed cases and 93 suspected dengue cases with twelve cases admitted, of which ten has been discharged. Dengue cases range in age from 8 months to 67 years, with the most affected group being those aged 20 to 39 years. Among all reported cases, 56% are female and 44% male. The serotypes that are responsible for this current outbreak are Dengue Virus Type 1 (DENV-1) and Dengue Virus Type 2 (DENV-2), tested at VIDRL in Melbourne. (Govt. of Kiribati, 20 May 2025)

As of 29 May 2025, the Ministry reported a total of 55 dengue confirmed cases and 225 suspected dengue cases with thirty-four cases admitted, of which twenty-nine have been discharged. Dengue cases range in age from 8 months to 84 years, with the most affected group being those aged 3 to 9 years and 20 to 39 years age group. Among all reported cases, 43% are female and 57% male. Nauru
Dengue cases were initially reported by the Republic of Nauru (RON) Hospital on 27 June 2025. As of 19 July 2025, the Ministry of Health reported 125 dengue confirmed cases of which 58% are males and 52% females. The most affected age group is the 5-10years with 75% of the total cases are less than 18years old. The youngest case was 11-month-old, and the oldest case was 64 years old. A total of 11 cases were reported hospitalised. The cases are distributed all over the island with majority of the cases from Meneng (25) followed by Denigomodu (20), Aiwo (14) and Baitsi (12). As of 24 July 2025, the Ministry of Health reported 200 dengue confirmed cases. Nearly 90% of these are children aged 1 to 14 years, with the highest concentration of cases reported from the districts of Meneng, Denigomodu (Locations settlement), and Aiwo. Nauru reported 1 confirmed Dengue Fever case on 10th September 2025 (SITREP 62), bringing the cumulative number of reported Dengue Fever cases to 447. From 11th to 14th September 2025, no new confirmed cases of Dengue Fever have been reported. Cumulatively, 113 admissions (no currently admitted cases) and 2 deaths have been reported. Palau
No new Dengue Fever cases were reported in this current week (epi week 34, 2025). From 1st July and 3rd August 2025, 7 cases of Dengue among residents had been confirmed. DENV-1 and DENV-2 have been detected in 2 of the submitted samples. (Govt. of Palau, 2 Sep 2025)

Samoa
As of 17 April 2025, the Ministry of Health has declared a dengue fever outbreak in Samoa with 15 confirmed dengue cases reported since January 2025 and one reported death. The 12-year-old boy died of haemorrhagic dengue fever after being medevacked to New Zealand. Most of the cases have been reported at Moto’otua Main Hospital particularly in the Northwest of Upolu with all age groups affected. As of 21 April 2025, the Ministry of Health reported six lab-confirmed dengue cases in EpiWeek 16; 14 – 20 April 2025, bringing the total confirmed cases to 21 and one reported death since the declaration of the outbreak on 17 April 2025. Most of the cases have been reported at Moto’otua Main Hospital particularly in the Northwest of Upolu with all age groups affected. Currently the dengue serotype is undetermined. As of 28 April 2025, the Ministry of Health reported eighteen lab-confirmed dengue cases in EpiWeek 17; 21 – 27 April 2025, bringing the total confirmed cases to 36 and one reported death since the declaration of the outbreak on 17 April 2025. 94% of the cases are reported from Upolu Island; 56% male and 44% female. The 15–19-year-olds are the most affected age group with 36% of the cases requiring hospitalization. 74% of the cases received vector control measures follow-up, which includes environmental assessments and outdoor residual spraying. The majority of cases (82%) are from Upolu Island, with 55% male and 45% female. Most cases are distributed across the Apia Urban Area and North West of Upolu regions, with individuals under 20 years old (86%) being the most affected. 10% of cases required hospitalisation, and of these admissions, 88% have been discharged. As of 14 July 2025, the Ministry of Health reported 331 lab-confirmed dengue cases in EpiWeek 28 (7 – 13 July 2025) bringing the total number of confirmed cases to 1276. This week the Ministry also confirmed the death of an 8-year-old child from Vailoa Faleata, marking the second fatality reported since the declaration of the outbreak on 17 April 2025. The majority of cases (81%) are from Upolu Island, with 55% male and 45% female. Most cases are distributed across the Apia Urban Area and North West of Upolu regions, with individuals under 20 years old (87%) being the most affected. 10% of cases required hospitalisation, and of these admissions, 76% have been discharged. As of 28 July 2025, the Ministry of Health reported 521 lab-confirmed dengue cases in EpiWeek 30 (21 – 27 July 2025) bringing the total number of confirmed cases to 2,277 and two reported deaths since the declaration of the outbreak on 17 April 2025. As of 17 August 2025, the Ministry of Health reported 9,571 clinically diagnosed dengue cases, of which 3,070 are lab-confirmed and six dengue-related deaths since the declaration of the outbreak on 17 April 2025. 72% of these cases are children under the age of 15 years. 50 cases are admitted with 1 case admitted to ICU. As of 24 August 2025, Samoa Ministry of Health has recorded a total of 10,982 clinically diagnosed dengue cases since January, including 3,379 lab-confirmed cases, with six dengue-related deaths. Children under 15 years account for 72% of cases, and the circulating serotypes remain predominantly DENV-1 (91%) and DENV-2 (9%). Case numbers dropped by 19% compared to the previous EpiWeek, with 1,307 new clinical cases and 309 lab-confirmed. Hospital admissions also declined by 14%, with 184 new admissions, 129 discharges, and 55 patients still hospitalised, none in ICU. This brings the cumulative number of confirmed cases (since January 2025) to 3,379. Majority of the confirmed cases (85%) are serotype 1. Children under 15 years account for 73% of cases, and the circulating serotypes remain predominantly DENV-1 (91%) and DENV-2 (9%). Case numbers decreased by 20% compared to the previous EpiWeek, with 1,043 new clinical cases and 251 lab-confirmed. Hospital admissions decreased by 11%, with 163 new admissions, 156 discharges, and 49 patients still hospitalised, none in ICU. Tonga
The Ministry of Health declared an outbreak of Dengue Fever (DF) for Tonga on 19 February 2025. [...] 42 new cases reported with positive rapid test and that brings up the total cases of DF to 52. Majority of confirmed cases from Vava’u (25), the rest from Tongatapu (17) and ‘Eua (10). Age group mostly affected is 10-19 years old. 10 cases are hospitalized, 2 in Vaiola Hospital and 8 in Prince Ngu Hospital. 3 of these cases are diagnosed with dengue hemorrhagic fever (DHF). As of today, 4 new cases reported with positive rapid test, that brings up the total DF cases to 73. Majority of confirmed cases from Tongatapu (33), the rest from Vava’u (28) and ‘Eua (12). Age group mostly affected is 10-19 years old. A total of 5 cases are currently hospitalized (4 Vaiola, 1 Niu’eiki), and all are stable. No dengue-related death reported so far. It is now confirmed that Dengue Virus Type 2 is responsible for the current dengue outbreak in Tonga according to the first batch of samples collected from Tongatapu. The result for the 2nd batch which included samples from Vava’u and ‘Eua is expected to be available later this week. Preventative activities to hot spot and high risk areas in collaboration with Tonga Red Cross Society, are ongoing with continuous technical support from WHO, UNICEF and SPC. Tonga’s Ministry of Health officially declared the end of the dengue fever outbreak on 20 August 2025, after two consecutive weeks with zero reported cases. As of 17 August 2025, 87 confirmed dengue cases have been reported. Of these, 71% have recovered, while 22 cases remain active, all located on Funafuti. A total of 10 cases were hospitalised, comprising 60% paediatric and 40% adult patients. Among the total cases, 52 are males and 35 females. The majority of cases are from the 5-14 years age group with the youngest being 3 months old and the oldest 54 years old.
