Pacific: Dengue Outbreak - Oct 2018

Disaster description

American Samoa

As of 16 February 2019, the Department of Health Surveillance Office has reported two cases of dengue in 2019. (Govt. of American Samoa, 16 Feb 2018)

Cook Islands

As of 25 February, four cases of dengue (serotype unknown) have recently been confirmed. Samples that are NS1Ag positive are being sent to a reference laboratory for further testing and serotyping. This is the first cluster of locally acquired dengue cases reported in the Cook Islands since 2016. (SPC, 25 Feb 2018)

On 27 February, the Ministry of Health has declared a dengue outbreak following the confirmation of seven dengue cases in Rarotonga in the past month. Of these cases, one was a visitor from French Polynesia with dengue serotype 3 (DENV-3), while the others were dengue serotype 1 (DENV-1). There are no dengue cases in the Pa Enua. As of 27 April, 39 dengue cases, including 18 confirmed DENV-1 cases and 21 probable cases were reported. A total of 69 dengue cases have been reported from 28 January to 30 June, of which 22 were confirmed, and 47 were probable cases (NS1Ag positives). 66 cases from the main island of Rarotonga, and 3 (confirmed Dengue-1) from Aitutaki. No cases reported from the rest of the outer islands. A total of 38 cases have been hospitalised. No deaths reported. (Govt. of Cook Islands, 10 Jul 2019)

As of 31 August, a total number of 83 dengue cases have been confirmed (22 confirmed dengue serotype 1, and 61 were confirmed NS1Ag positive). Majority of the cases are from Rarotonga. The northern part of the island, and the airport area were mainly affected. Three (confirmed DENV-1) from Aitutaki island. There are no cases reported from the rest of the Outer Islands. From the beginning of the outbreak, a total of 44 cases have been hospitalised and discharged. There have been no reported deaths attributed to dengue infection. (Govt. of Cook Islands, 9 Sep 2019)

As of the end of September 2019, a total number of 91 cases have been confirmed (22 confirmed dengue serotype 1, and 69 were confirmed NS1Ag positive). Aitutaki dengue count still remain at 3 cases. The northern part of the island of Rarotonga, is still the hot spot, with 70% of the total cases reported on Rarotonga; there are no cases reported from the rest of the Outer Islands. A total of 46 cases have been hospitalised and discharged. The cases were also given free mosquito nets to use at home. From the beginning of the outbreak to now, there have been no reported deaths attributed to dengue infection. (Gov't of Cook Islands, 11 Oct 2019)

Cook Islands reports two imported dengue serotype-2 cases from travellers from French Polynesia. There is an ongoing dengue serotype-1 outbreak. As of 31 October there have been 113 cases, there have been no deaths reported. In the period between 28 January to 30 November 2019, a total of 120 dengue cases were recorded. More than 95 percent of these cases were reported on the island of Rarotonga. Seven dengue cases were recorded in the month of November. A total of 48 cases have been hospitalised and discharged since the beginning of the outbreak. With more than 20 confirmed and/or suspected contractions reported between January and November, those aged 10-19 years old were the most afflicted by the outbreak. The female to male ratio of reported dengue cases is 1:1.4, respectively. (Govt. of Cook Islands, 6 Dec 2019).

Eleven (11) probable cases have been reported in January 2020, an increase from 6 in December 2019. From 28 January 2019 to 31 January 2020, a total of 137 cases have been recorded, including confirmed and probable cases (Pacific Community, 10 Feb 2020)

From 28 January 2019 to 12 March 2020, a total of 195 cases have been recorded, including confirmed and probable cases. January 2020 to 12 March 2020: 69 cases including 4 DENV-1, and 2 DENV-2. Feb 2020: 2 new DENV-2 cases, with no travel history. (Govt. of Cook Islands, 13 Mar 2020).

French Polynesia

On 2 April, the Ministry of Health of French Polynesia reported two autochthonous DENV-2 cases confirmed in week 11 (week ending 17 March 2019). To date this year, three DENV-2 cases have been reported, including an imported case from New Caledonia reported in February 2019. (WHO, 4 Apr 2019)

One new case of dengue serotype-2 reported outside Taunoa, where two indigenous cases of dengue serotype-2 were previously reported, the first case reported at the end of March. Dengue serotype-2 was last identified in French Polynesia in 2000. [Source: MoH and Media] (WHO, 11 Apr 2019)

During weeks 35 and 36 in 2019, there were 7 DENV-1 and 254 DENV-2 cases reported. An outbreak of DENV-2 was declared on 15 April 2019 in Tahiti and a total of 834 indigenous cases of DENV2 have been reported from week 6 to week 36. Among these, most cases have been reported from Tahiti (744) and others form Moorea (40), Bora Bora (14) and Nuku-Hiva (11). (WHO, 26 Sep 2019)

During weeks 37 and 38 in 2019, there were 14 DENV-1 and 249 DENV-2 cases reported. An outbreak of DENV-2 was declared on 15 April 2019 in Tahiti and a total of 1058 indigenous cases of DENV-2 have been reported from week 6 to week 38. Among these, most cases have been reported from Tahiti (924) and others form Moorea (51), Bora Bora (21) and Nuku-Hiva (11). (WHO, 10 Oct 2019)

During weeks 39 and 40 in 2019, there were 14 DENV-1 and 249 DENV-2 cases reported. An outbreak of DENV-2 was declared on 15 April 2019 in Tahiti and a total of 1,488 indigenous cases of DENV-2 have been reported from week 6 to week 40. Among these, most cases have been reported from Tahiti (1,259) and others form Moorea (68), Bora Bora (29) and Nuku-Hiva (11). (WHO, 24 Oct 2019)

During weeks 41 and 42 in 2019, there were 2 DENV-1 and 346 DENV-2 cases reported. Since the DENV-2 outbreak was declared 15 April 2019 in Tahiti, most dengue cases have been reported in Tahiti (1,420), followed by Moorea (70), Bora Bora (34 ) and Nuku-Hiva (13). (WHO, 7 Nov 2019)

As of 9 February, the authorities reported 2,649 DENV-2 cases from the beginning of 2020. Of the total number of cases reported in 2020, two were imported cases. (Pacific Community, 25 Feb 2020)

Guam

In a press release on 12 September 2019, the Guam Department of Public Health and Social Services confirmed the first case of dengue fever-dengue serotype-3. This is the first locally acquired dengue case detected and reported on Guam in the last 75 years. [Source: Guam Dept of Health and Social Services]. (WHO, 13 Sep 2019)

As of 17 September, the Guam Department of Public Health and Social Services (DPHSS) confirmed that a second person with locally acquired dengue infection was identified, after being seen at the Guam Memorial Hospital ER. The individual is an adult male who resides along Swamp Road, Dededo. (Gov't of Guam, 17 Sep 2019)

As of 18 September, the Guam Homeland Security/Office of Civil Defense (GHS/OCD) Emergency Operations Center (EOC) was partially activated at 2 p.m. Specific government of Guam agencies have been called to assist the EOC to facilitate greater collaboration and communications between government agencies. The Joint Information Center (JIC) activated to support the response plan.The Epidemiology and Surveillance Team visited a total of 90 homes in both Mangilao and Dededo. (Gov't of Guam, 19 Sep 2019)

As of 23 September, ​seven​ confirmed cases of locally acquired dengue infection and​ four confirmed cases of imported dengue infection have been reported by Guam Department of Public Health and Social Services (DPHSS). (Pacific Community, 23 Sep 2019)

Between 21-27 October, Guam identified one new locally acquired case. That brings the total dengue cases identified to 18 (11 locally acquired and 7 imported). (WHO, 27 Oct 2019)

Kiribati

Marshall Islands

On 6 August, the Republic of Marshall Islands (RMI) declared a State of Health Emergency due to dengue outbreak on Ebeye island. There has been one confirmed dengue serotype-3 and 22 probable cases on 1 August in Ebeye. The last dengue outbreak (serotype-4) on RMI was in 2011, where there were 1,603 cases; including 867 laboratory confirmed cases. (WHO, 4 Aug 2019)

As of 30 August, there are 16 confirmed (RT-PCR) by Hawaii State Laboratory and 276 dengue-like illness cases. 219 cases in Ebeye (attack rate = 18.4 per 1,000 population), 57 cases in Majuro (attack rate 2.2 per 1,000 population). Of the confirmed cases 11 were from Ebeye and 5 cases were from Majuro. There have been no deaths reported. (WHO, 3 Sep 2019)

As of 8 Sept there have been 344 suspected cases, including 122 lab confirmed cases. Of the 344 cases, 228 of the cases are from Ebeye and 124 of the cases are from Majuro . The attack rates are 20 per 1,000 population for Ebeye and 4.7 per 1,000 population for Majuro. The overall attack rate for RMI is 6.6 per 1,000 population. As of 22 September, 535 suspected cases have been reported in Ebeye and Majuro. The number of reported cases is declining in Ebeye and no cases have been reported yet in the outer atolls. (Pacific Community, 30 Sep 2019)

As of 29 September, there have been 607 suspect cases of which 161 have been lab confirmed. This is outbreak week #20 in Ebeye and #7 in Majuro. No cases yet in outer atolls. As of 20 October, there have been 772 dengue like illness of which 220 have been lab confirmed. This is outbreak week #23 in Ebeye, #11 in Majuro and #1 in Utrik. 4 confirmed cases in the Outer Islands. Symptom and age profile of cases is typical for dengue fever. As of 27 October, there have been 914 suspected dengue cases, of which 280 have been lab confirmed. The cases have been found in the islands of Ebeye, Majuro, Utrik, and for the first time during the outbreak, Aur. As of 10 November, there have been 1,085 dengue-like illness cases. Dengue serotype-3 is in circulation. The outbreak is slowing down in Ebeye, continuing in Majuro, with cases reported from Rongrong and Utrik. As of 1 December, the Marshall Islands has seen 964 suspected dengue cases. 431 dengue cases have been confirmed. At the time of writing, Ebeye, Majuro, Utrik and Aur have all reported cases. To date, there has been one death attributed to dengue, and one patient who was evacuated out-of-country for treatment. (Govt. of the Marshall Islands, 1 Dec 2019)

There have been 1,635 dengue like illness of which 533 have been laboratory confirmed by the Republic of the Marshall Islands as of 22 December 2019. One death has been recorded. According to the Government of the Republic of the Marshall Islands, as of 29 December 2019, there have been 1,735 dengue-like illnesses reported to authorities. More than 550 have been confirmed cases, while one death has been attributed to the outbreak. The dengue outbreak continues in Majuro. (Pacific Community, 6 Jan 2020).

As of 2 February, there have been 2,456 dengue like illness, of which 1,203 have been laboratory confirmed as per the government of the Republic of the Marshall Islands. The outbreak still continues in Majuro. (Pacific Community, 10 Feb 2020)

RMI Dengue serotype-3 outbreak is ongoing (week 43) in Majuro in the Republic of Marshall Islands. As of 8 March 2020, there have been 3,121 dengue cases, 1479 were laboratory confirmed and 2 deaths reported since June 2019. 90 new cases were seen in the last 7 days, 76% of cases were from Majuro. Micronesia (Federated States of)

As of 30 August, there are a total of 406 dengue-like illness cases of which 91 cases were reported dengue Rapid Diagnostic Test positive. The weekly number of cases is increasing. Dengue serotype-3 was identified by the Guam Public Health Laboratory. There were 14 reported co-infection cases with leptospirosis. There have been 32 hospitalised cases and one death reported. A total of 77 new DLI cases were seen during epi week 37 (34% increase from week 36) bringing the total of 619 cases since January 1, 2019. 22 cases had positive RDT this week totaling 158 RDT positive cases (146 NS1 and 12 IgM). Majority of RDT positive cases (81%) are children under the age of 20 years old. The overall attack rate for Yap is 16.3/1000. The highest attack rate (5.5% ) are among the age group10-14 years. 10 cases were admitted, bringing the total of admissions up to 56 in all. A total of 66 new DLI cases were seen during epi week 38 (11% decrease from week 37) bringing the total of 685 cases since January 1, 2019. 29 cases had positive RDT this week bringing the total positive RDT of 187 (172 NS1 and 15 IgM). Majority of RDT positive cases (78%) are children under the age of 20 years old. The overall attack rate for Yap is 15.6/1000. The highest attack rate (6.6% ) are among the age group 10-14 years. 12 cases were admitted, bringing the total of admissions up to 68 in all. Out of the 12 admissions, 6 cases had DHF bringing the total of DHF up to 35. Still one death is associated with dengue outbreak. (Gov't of the Federated States of Micronesia, 24 Sep 2019)

A total of 75 new DLI cases were seen during epi week 39 (14% increase from week 38) bringing the total of 760 cases since January 1, 2019. Twenty cases had positive RDT this week bringing the total positive RDT of 207 (191 NS1 and 16 IgM). Rehydration visits this week was 22, bringing the total up to 521. There is one death this week due to dengue and leptoco-infection. Samples have been collected for confirmatory testing. As of 1 October, there have been two deaths attributed to this dengue outbreak (CFR 1%). (Gov't of the Federated States of Micronesia, 1 Oct 2019)

A total of 66 new DLI cases were seen during epi week 40 (12% decrease from week 39) bringing the total of 826 cases since January 1, 2019. 34 cases had positive RDT this week (70 % increase from week 39) bringing the total positive RDT of 241 (222 NS1 and 19 IgM). 11 cases were admitted, bringing the total of admissions up to 86 in all. Out of the 11 admissions, 4 cases had DHF bringing the total of DHF up to 44. There was no admission cases with leptospirosis coinfection in week 40. Rehydration visits this week was 85, bringing the total up to 606. As of 9 October, there have been two deaths attributed to this dengue outbreak. In Yap State, a total of 67 new cases were seen during epi week 41 (1.5% increase from week 40) bringing the total of 893 cases since January 1, 2019. 23 cases had positive RDT this week (32% decrease from week 40) bringing the total positive RDT of 264 (243 NS1 and 21 IgM)There were 10 cases were admitted in epi week 41, bringing the total of admissions up to 96 in all. There were 5 cases were admitted in epi week 43 (21-27 October), bringing the total of admissions up to 101 in all. There have been two deaths related to the outbreak. (WHO, 27 Oct 2019)

As of week ending 3 November there were 77 dengue-like illness cases, bringing the total to 1,071 cases since 1 January 2019. The weekly number of cases appears to be plateauing. Nauru

An increasing number of dengue positive cases (serotype unknown) has been reported in Nauru. From 25 October to 5 December 2018, 114 samples were tested in the laboratory using NS1Ag Combo and 42 were positives. (Pacific Community, 10 Dec 2018) However, as of 4 April 2019, no new cases of dengue fever have been reported in Nauru over the past 4 weeks. (Pacific Community, 8 Apr 2019)

New Caledonia

A new outbreak of dengue serotype 2 (DENV-2) has been declared in New Caledonia with 1,995 confirmed or clinical dengue cases from January 1, 2018 to December 19, 2018. (Govt. of New Caledonia, 21 Dec 2018)

The ongoing outbreak is mainly in Port Vila. Of the 8 samples sent to Medipole, New Caledonia 6 are positive for Influenza B Victoria lineage. A press release has been released regarding the situation, refresher training has been conducted for clinicians at the Vila Central Hospital and surveillance has been strengthened. As of 26 September, there have been a total of 3,898 dengue cases including two deaths reported since the beginning of 2019 (Figure 10). The number of clinically-diagnosed and confirmed cases reported weekly continues to decrease. Of the 306 samples serotyped in 2019, 304 are DENV-2 and there have been two imported cases including one DENV-1 and one DENV-4. (WHO, 10 Oct 2019)

As of 16 October, there have been a total of 3,902 dengue cases including two deaths reported since the beginning of 2019 (Figure 10). The number of clinically-diagnosed and confirmed cases reported weekly remains low and the epidemic was official declared over during week 34. Of the 310 samples serotyped in 2019, 308 are DENV-2 and there have been two imported cases including one DENV-1 and one DENV-4. (WHO, 24 Oct 2019)

Northern Mariana Islands

As of 6 September, the Commonwealth Healthcare Corporation (CHCC) confirmed two dengue fever cases. There is still no evidence on island transmission has occurred and the CHCC believes the chance of an outbreak remains low. (Gov't of the United States of America, 20 Sep 2019)

Palau

On 7 December, the Palau Ministry of Health declared dengue outbreak following the first ever laboratory confirmation of two cases with dengue serotype-3. (WHO, 9 Dec 2018)

As of 20 December, 27 confirmed cases were reported in Palau. The majority of cases reside in the most populous state, Koror, in various hamlets. Most cases are within 20-29 years of age. There were seven (7) patients (3 children and 4 adults) currently hospitalized for treatment of dengue fever. There have been no reported deaths attributed to dengue infection. As of 28 May, there were 9 new cases reported during May 20 - 26, 2019. This brings the total from December 1, 2018 to May 26, 2019 to 196 cases. (Govt. of Palau, 28 May 2019)

Between June 10 June 16, 2019, there were 15 new cases reported. This brings the total from December 1, 2018 to June 9, 2019 to 250 cases. (Govt. of Palau, 18 Jun 2019)

The Palau Ministry of Health reports that between August 5-11, 2019, there were 29 new cases. This brings the total from December 1, 2018 to August 11, 2019 to 486 cases. There has been one death attributed to dengue infection during this outbreak. (Gov't of Palau, 13 Aug 2019)

According to the Palau Ministry of Health, 26 new cases were reported between August 26 – September 1, 2019. This brings the total from December 1, 2018 to September 1, 2019 to 570 cases.There have been two deaths attributed to dengue infection during this outbreak. (Gov't of Palau, 01 Sep 2019)

The Palau Ministry of Health reports that between September 2 – 8, 2019, there were 16 new cases reported. This brings the total from December 1, 2018 - September 8, 2019 to 586 cases. There have been two deaths attributed to dengue infection during this outbreak. (Gov't of Palau, 12 Sep 2019)

Between 20 and 26 January, 5 new dengue cases were reported. This brought the total number of cases for the month of January 2020 to 35. From December 2018 until January 2020, a total of 802 cases had been reported in Palau. (Pacific Community, 10 Feb 2020)

Tuvalu

Tuvalu declared a dengue outbreak in epi week 12 (week ending 31 Mar 2019). There were 15 15 Dengue Duo RDT positive cases as of Aprl 4, 2019 (WHO, 11 Apr 2019). By May 2019, the number of suspected cases rose to 137, with 51 Dengue Duo RDT positive cases. (WHO, 9 May 2019). As of 26 June 2019, there were a total of 476 dengue like illness cases including 211 laboratory confirmed cases. There have been 2 deaths associated with this current outbreak bringing the case fatality rate to 0.4%. About 45% of cases are in the <10 year old age groups. From March 25 to August, a total of 517 dengue like illness (DLI) were reported and 210 were confirmed. Eight DLI cases were reported in week 32 in Funafuti and two of them were confirmed. Six of the new DLI cases, including the two confirmed, have travel history to Vaitupu in Central Islands group (Govt. of Tuvalu, 15 Aug 2019)

Vanuatu

Between January 1st and April 16th 2019, there have been a total of 216 suspected dengue cases reported including 18 confirmed cases in Vanuatu. Between 8th and 16th April, 42 suspected dengue cases reported in Port Vila and Luganville and 4 cases were confirmed positive for dengue in Luganville only. One of the confirmed cases of DENV-2 in Port Villa was an imported case from a traveler coming from the Solomon Islands in January 2019. One confirmed case serotype unknown with travel history to New Caledonia was detected in Luganville on the 15th of April 2019. 13 confirmed cases were locally acquired in Luganville, Santo island. Wallis and Futuna

On 27 May, a health alert on dengue was issued by the Wallis and Futuna Islands Territory Health Agency following reports of two new cases of dengue in Wallis. [Source: Government] (WHO, 30 May 2019)

On 21 November, Wallis and Futuna has declared a dengue (DENV-2) outbreak (SPC, 26 Nov 2019)
