WHO is supporting the Ministry of Public Health of Madagascar to respond to an unusually large measles outbreak. Madagascar last experienced measles outbreaks in 2003 and 2004, with reported number of cases at 62 233 and 35 558, respectively. Since then, the number of reported cases had sharply declined until the current outbreak. From 4 October 2018 to 7 January 2019, 19 539 measles cases and 39 “facility-based” deaths (case fatality ratio: 0.2%) have been reported by the Ministry of Public Health (MoH) of Madagascar.

  • The Ministry of Public Health in Madagascar formally declared a measles outbreak on 26 October 2018. No deaths have been reported officially to date, although mortality probably related to measles has been reported from various hospitals. The most affected district is Antananarivo Renivohitra. As of 14 December more than 9 364 cases have been officially reported from 55 districts.

HIGHLIGHTS

  • 9.6 million people were severely food-insecure in Southern Africa at the start of the lean season (October 2018- April 2019)

  • Three districts in Zimbabwe and two in Madagascar are facing Emergency food insecurity (IPC phase 4) due to extreme loss of livelihoods

  • There is at least an 80 per cent chance of an El Niño phenomenon between October and December 2018, which could exacerbate the deteriorating situation.

Highlights

• A seasonal plague upsurge, predominantly of the bubonic form, usually hits Madagascar annually between September and April. Similar to last year, this year’s plague season has begun earlier than usual. The current outbreak is predominantly in rural areas and is largely contained. As of mid–August a cumulative total of 103 plague cases (38 confirmed and 65 suspected) have been recorded and 10 deaths.
