Tackling the problems of poverty, vulnerability and exclusion that persist in parts of the world that continue to be affected by violence or political insecurity is difficult for several reasons. For one, because of the complexity of the prevailing social, economic and political systems, solutions to chronic problems are far from obvious.
One response to this aspect of the challenge is adaptive programme design and management.
his report takes its inspiration from the United Nations Secretary-General’s Every Woman Every Child initiative, which calls for countries to do everything possible to protect the lives and futures of all women, children and adolescents. It follows the approach used for the
State of the World’s Midwifery 2014 report, but focuses on 21 of the 23 countries in the United Nations Population Fund (UNFPA) East and Southern Africa region.
• Many countries across the African continent face recurrent complex emergencies, frequent food insecurity, cyclical drought, and sudden-onset disasters such as earthquakes, floods, and storms. In FY 2016, as in previous years, USAID/OFDA not only responded to urgent needs resulting from disasters, but also supported DRR programs that built resilience and improved emergency preparedness, mitigation, and response capacity at local, national, and regional levels.
Gender inequality is costing sub-Saharan Africa on average $US95 billion a year, peaking at US$105 billion in 2014– or six percent of the region’s GDP – jeopardising the continent’s efforts for inclusive human development and economic growth, according to the Africa Human Development Report 2016.

Objectives and activities
In 2015, as the Millennium Development Goals reach their deadline, the world can reflect on real progress. Since 1990, thanks to the actions of millions of people around the globe, extreme income poverty has been cut by almost two-thirds, child mortality has fallen by more than half, and more children are attending primary school than ever before.
But these achievements tell only part of the story.

Afghanistan: The number of severely food insecure has risen to 1.5 million people, according to a new assessment. 7.3 million people are moderately food insecure. Food security among IDPs is worsening, with around 200,000 people reported to be in need of immediate assistance.
Syria: Only 41% of Syria’s public hospitals are fully operational. The latest in a number of local truces around Damascus has been agreed between state forces and opposition in Qadam. 191,369 people were reported killed March 2011–April 2014, mainly in Rural Damascus, Aleppo, Homs, Idleb, Dar’a and Hama, according to new UN figures.
Pakistan: Water, sanitation, and health services are urgent needs among the 780,000 registered displaced from North Waziristan (government figures). The data is being cleaned to check for duplication.
Iraq: Access to areas within the governorates of Anbar, Babylon, Diyala, Salah al Din, Kirkuk, and Ninevah remains difficult due to ongoing violence clashes, disruption of communication and transportation routes, and a widespread shortage of fuel.
Iraq: The Islamic State of Iraq and the Levant and its allies have overrun non-Kurdish controlled areas Ninevah and significant areas of neighbouring Salah al Din and Kirkuk governorates, as well as part of northern Diyala. Access is severely limited, and hundreds of thousands of displaced persons are in need of humanitarian assistance.
Syria: Fierce fighting between opposition groups has led to further displacement in Deir-ez-Zor while a truce between the Government and the opposition is expected to see the Old City of Homs handed over to the Government after its recent campaign to retake it.
The present report, which covers the period from January to December 2013, is submitted pursuant to paragraph 22 of Security Council resolution 2106 (2013) , in which the Council requested me to submit annual reports on the implementation of resolutions 1820 (2008) , 1888 (2009) , 1960 (2010) and 2106 (2013) with regard to conflict - related sexual violence. The report presents information on parties to conflict credibly suspected of committing or being responsible for acts of rape and other forms of sexual violence.
Snapshot 16–23 April
Central African Republic: Renewed clashes between anti-balaka and Seleka fighters occurred in several locations in central, northern, and southwestern provinces. MISCA soldiers are being redeployed to cover areas previously secured by the Chadian contingent. A Christian religious leader was killed and four were abducted in two northern towns, while 30 Christian civilians were reportedly killed in a central town.

Efforts to build lasting peace at regional level involve multiple strata of actors, institutions and processes. Regional organisations that are in search of shared peace and security must take cognisance of the various layers of political and social history of many sovereign states. These bodies must accommodate different economies, legal systems and security capacities, cultures and confl icting interests. Challenges such as these are particuraly acute for new regional institutions, especially when they ambitiously set out to solve newly-defi ned problems.
