Somalia Health Cluster Bulletin #42 - December 2010

HIGHLIGHTS

- In 2010, a total of 7,634 weapon-related injuries were reported from three hospitals in Mogadishu.

- Of 2,522 cases of suspected malaria in December (in Banadir, Lower and Middle Shabelle, Bay, and Hiraan regions), 56% (1,412) were confirmed by either laboratory or rapid diagnostic tests (RDT).

- This month, 2 out of 18 samples for whooping cough in Lower Shabelle tested positive.

SITUATION OVERVIEW

The evolution of an epidemic cannot be defined with precision. Past experience has shown that an epidemic does not follow a uniform pattern but consists of multiple outbreaks in different areas that may peak in a few weeks in some places, a few months in others. For planning purposes, the Haitian government is using an estimate of 400,000 cases over the first 12 months, with half of those cases in the first 3 months. There are concerns that in some rural areas the case fatality rate is still very high.

Since the beginning of December, the response has focused on keeping the supply chain operational and developing contingency plans for new emergencies. WHO/PAHO, the Cuban Medical Mission and the US Centers for Disease Control (CDC), in support to the Ministry of Health and Population (MSPP), have developed a plan dedicated to surveillance, alert and response and departmental coordination which will lead to a more efficient response in situations where security is volatile.

Contingency planning in Côte d'Ivoire

28 December 2010 -- Fearful of the volatile situation in the country, thousands of people from Côte d'Ivoire have already sought refuge in Liberia, Guinea and Ghana. In response to WHO's request, the Government of Italy has authorized the dispatch of emergency medical supplies and equipment from the warehouse in Brindisi to the UN humanitarian depot in Accra. These supplies cover the basic health needs of 70 000 people for 3 months (including malaria) and the specific needs of 500 patients requiring trauma surgical care. They will support WHO's contingency plan

SITUATION OVERVIEW

The latest data indicates that about 120,000 cases have been reported and 2,400 persons have died. The number of cases continues to increase but the case fatality rate is decreasing. This seems to indicate that the measures being implemented are starting to have effect but it is clear that there is still a lot of work to do. The main current concerns are the outbreaks in remote rural areas.

Surveillance summary and analysis of selected performance indicators for measles/rubella surveillance-Iraq 2010; first 48 weeks compared with all 2009

It is quite clear that around 8% of confirmed measles cases are still occurring among infants below the age of vaccination, while 49% of confirmed measles cases occur in the age group below 3 years of age. MoH has completed an H to H immunization national campaign to immunize children 6 months to 3 years of age using mono-valent measles vaccine. This decision is quite wise since 41% of measles cases fall within the target age group

33% of cases are occurring among 1-5 year old children which points to several weaknesses in routine and SIAs.
